Standard titration schedule
Semaglutide requires gradual dose escalation to minimize GI side effects. This is the schedule used in Syntho protocols, matching the FDA-approved Wegovy titration:
Starting dose. Mild appetite changes, possible light nausea.
First real appetite suppression. Early weight loss begins.
Significant reduction in food cravings.
Approaching full therapeutic effect.
Maintenance. Continue at this level unless side effects require adjustment.
Each phase can be extended. If nausea or GI symptoms persist beyond 2 weeks at any dose, stay at that level for an additional 2-4 weeks before increasing. Going slower is standard practice, not a failure.
Reconstituting semaglutide
Compounded semaglutide arrives as lyophilized powder. Add bacteriostatic water to create an injectable solution. The process is identical to any peptide reconstitution.
Preparation
- Clean workspace, sterile gloves
- Warm vials to room temperature
- Swab both vial tops with alcohol
Mixing
- Inject water against the glass wall at 45 degrees
- Gently swirl until completely clear
- Label with date, concentration, and discard date
Storage: Refrigerate at 2-8 degrees C. Stable for approximately 28 days with BAC water. Keep upright, protected from light. Do not freeze.
Compounded vs. brand-name
The semaglutide molecule is identical in both compounded preparations and FDA-approved products (Wegovy, Ozempic). The titration schedule, dose amounts, and timing are the same.
The difference is delivery: brand-name products come in pre-filled pens with fixed concentrations. Compounded semaglutide comes as lyophilized powder that you reconstitute yourself, giving you control over the concentration and draw volume.
